Outreach Support Service – Stone Hill School
At Stone Hill we offer an Outreach Support Service to support mainstream pupils who are presenting with Social Communications Difficulties. This is collaborative support, between Stone Hill and the school requesting support. Below is information regarding the joint agreement.
Joint Agreement following acceptance of a request for support and signed Parental Consent
1. Stone Hill Outreach Support Service will:
• Contact you to acknowledge receipt of your request and clarify any information on your Request Form and/or request further supporting information
• Arrange an initial visit: this may be to discuss the request with school/setting staff and/or parents or may be an initial observation of the child
• Discuss and share a signed and dated Record of Visit for every visit and agree next steps
• Where further assessments/ observations are undertaken these will be recorded and shared with school to share with parents
• Write a final Record of Visit to confirm either that:
a) school/setting staff are now able to meet needs without further support;
b) school has been sign posted to alternative services for further specialist support or
c) where longer term recommendations are advised these will be set out in an Assessment and Recommendations Summary Report.
2. The School will provide:
• A named contact and access to any other school staff who may be involved
• Any additional information if requested
• A suitable place to meet and where required to work with the child
• Details of any planned multi agency meetings and/or review meeting with parents
• Attend meetings with Outreach Support and sign the Record of Visit
• Complete the Recommendations Monitoring Sheet if required
• Facilitate any staff training as advised
• Share information with parents/carers and other practitioners involved and seek feedback as part of the evaluation

REquest a Visit to stone hill school
Stone Hill School is for children starting Y1 through to Y14. We are based on a mainstream setting; Lower School (KS1&2) uses the mainstream primary school model, Upper School (KS3&4) mirrors a secondary setting and our Post 16 (KS5) is designed to support independence towards employability. Nevertheless, we support children and young people with Cognition and Learning needs, where Moderate Learning Difficulties are the main barriers to their educational requirements. We have high expectations of all our pupils, supporting everyone in school in accessing the appropriate provision to meet their long term and medium term outcomes. We are not the right school for every child, and there are other schools to consider depending on your child’s primary need.
I recognise that it is a difficult decision to choose the right school; I hope that this form will help in the decision making, before coming for a visit.
